Forms

Students in the College of Science may request special modifications to their schedules, class grading options, or majors. Some of these tasks are handled at the college level. These include:

• SCBU, CHBU, Actuary, MATH/Business request  (Starting in the Fall of 2023, the Science-Business, Chemistry-Business, and Math/Business majors will no longer be available to incoming students. This form will be removed at the end of the 2022-23 academic year.)
• 10000-level course request (handled by first year of studies)
